Both PPO plans are administered through Anthem and include Prescription Drug Coverage.

Dental Insurance

Our comprehensive Dental Insurance is a PPO plan which is administered through MetLife.
- Preventive Procedures (i.e. exams, cleanings) 100%
- Basic Procedures (i.e. fillings, stainless steel crowns) 90%
- Major Procedures (i.e. bridges, dentures) 60%
Life Insurance
Life Insurance & AD&D
- Life and AD&D is Company Paid!!
- 100% of annual salary to a maximum of $150,000
Voluntary Life Insurance
- The first $10,000 is Company Paid!!
- Employees may choose to purchase benefits in $10,000 increments to a maximum of $300,000
- Employees may choose to purchase benefits in $10,000 increments to a maximum of $300,000
- Proof of good health is NOT required for up to $50,000 in Voluntary Life if elected within 30 days of hire
- Election of Voluntary Life Coverage for those other than new hires will require proof of good health
- Employees may choose to cover Eligible Spouses in $5,000 increments up to a maximum of $100,000
- Employee coverage is required for spouse to elect coverage
- Eligible children 14 days of age or order, employees may elect coverage in the amount of $2,000 or $5,000
- Child benefits cannot exceed 100% of the employee’s coverage
Disability Insurance
Short Term Disability
- Short Term Disability is Company Paid!!
- Benefits Percentage is 60% of pre-disability earnings
- Maximum weekly benefit is $1,500
Long Term Disability
- Long Term Disability is Company Paid!!
- Benefits Percentage is 60% of pre-disability earnings
- Maximum monthly benefit is $5,000
Section 125
The SBS FSA Program is administered through Paychex and this allows employees / participants to set aside pre-tax earnings (resulting in a substantial payroll tax savings) that may be used to pay for qualified medical and childcare assistance expenses.
